Beginning January 1, 2022, health care providers are required to estimate the cost of services for patients who are self-paying or not using their insurance benefits because the provider is out-of-network.
This means I will provide you with a detailed list of expected charges for services, called a Good Faith Estimate (GFE).
The GFE will likely overestimate the cost of services to provide you with the maximum expected out of pocket cost for treatment. The estimate is based on information known at the time it was created. If additional services are recommended, a new estimate will be presented to you. Estimated costs are valid for 12 months after the date of the GFE. This is not a contract and signing it does not obligate you to obtain the services estimated. Please keep a copy of the GFE for your records.
The GFE may not include any unknown or unexpected costs that may arise during treatment. Actual items, services, and charges may differ from the GFE. If this happens, federal law allows you to dispute (appeal) the bill.
If you are billed more than the GFE, you may:
Contact the provider to discuss the difference between the bill and the GFE. You may ask to have the bill reflect the GFE, ask to negotiate the bill, or ask if financial assistance is available.
Start a dispute resolutions process with the U.S. Department of Health & Human Services (HHS). You must start the dispute process within 120 calendar days of the date on the original bill. There is a $25 fee to use the dispute process. If the agency reviewing the appeal agrees with you, you pay the price on the GFE. If the agency agrees with the provider, you pay the higher amount.Â
You can learn more about Good Faith Estimates and how to appeal a bill at www.cms.gov/nosurprises or call HHS at 1-800- 985-3059.
